Periodontal (Gum) Treatment

Periodontal Disease is an infection of the teeth, gums and the bones around the teeth. And it’s the leading cause of a tooth loose. Stabbings condition begins with through diagnosis. The main cause of periodontal disease is the accumulation of plaque, the sticky film of food and bacteria that forms constantly on your teeth. The bacteria and plaque invade the spaces between the teeth and gums and begin producing toxins. These toxins combine with your body’s reactions to them destroys the bone around your teeth. Once bone has been lost, it never grows back on its own. When too much bone is lost, there is so little support for teeth. They get loose and have to be removed. If you have periodontal disease, you may have some of the symptoms.

Symptoms of Perio Disease
  • Persistent bad breath
  • Bleeding Gums
  • Swollen, tender gums.
  • Gums pulling away from teeth.
  • Loose teeth.

Healthy gums are tied against the teeth. There aren’t any pockets and they don’t bleed. It is also possible to have no noticeable symptoms when you have Periodontal Disease. In fact most people who have periodontal disease aren’t even aware of it. Because of this we perform a thorough examination with periodontal probe and X-Rays. Periodontal Probing measures the depth of the space in your gums. When you have periodontal disease, these spaces are called pockets. We measure the pockets from the bottom of the pockets where they attached of the tooth to the top of the gums.  If probe reading has more than three millimeters, then this is a sign of periodontal disease. In general the deeper the plaques, the greater the spread of periodontal disease. Gum sometimes bleeds during probing. This is also a sign of infection. X-Rays tell us a lot about periodontal disease. In advanced periodontal disease bone levels are much lower than they are in healthy mouth.

Dental Crowns and It’s Treatment Procedure


As you age, your teeth age too. Decay, Fillings, trauma or even just normal chewing habits can make your teeth break down over time. A porcelain dental crown or a cap is a cover that is placed over a tooth to protect a weakened tooth from further fracturing or deteriorating. A Porcelain crown could also be used to replace a discolored or badly shaped tooth or protect a tooth that has had a root canal. Lastly, a new porcelain crown is necessary to simply replace an old worn down or defective crown.

It normally takes two visits for your dentist to complete your Crown. On your first visit, a local anesthetic is administered to make the procedure more comfortable. During this visit your dentist will use a dental hand piece to shape and prepare the tooth so the crown can properly fit over it. In some instances tooth may need an additional procedure to replace areas of decay, oral fillings to better support the crowns. Next an impression of the tooth is taken and a temporary crown is inserted to protect the prepared tooth. This impression is then sent to a dental laboratory where custom fit permanent crown is made. In some cases on your second visit, the permanent crown is placed and if necessary adjusted to create a proper fit. If the fit, shape and color of the crown are satisfactory, your dentist will proceed with permanently cementing it into place.

Keep in mind in some instances the crown received back from laboratory may not fit as expected. In these cases your dentist may need to take a second impression or re-adjust the tooth in order to fabricate a crown that fits just right. After the successful completion of the crown it is common to feel some discomfort, mild pain when you chew or sensitivity to temperature changes for a few days. As advised by your dentist you can take over the counter pain medicine to alleviate the discomfort. If this symptoms persist more than a few days, Visit your dentist.

Finally, if your newly cemented crown feels too tall or your bite does not feel right, you should contact your dentist for further adjustment of the crown.

You may ask, Can’t I get a filling instead? The answer is No! When a tooth requires a crown, it is already been weaken to the point that it may not be strong enough to support a Filling. Using an unsupported Filling instead of a Crown often causes the tooth defracture beyond the point of repair.

Please keep in mind that the longevity and durability of your crown is directly related to your oral hygiene and the material selected to make the crown. All porcelain crowns are generally considered the most aesthetic choice since they closely resemble the color and appearance of your teeth. Porcelain continues to improve and in fact, there are newly developed porcelain materials which can better withstand the forces of chewing. Teeth Cleaning Procedure


Most people have heard of Gingivitis but you’ve probably never heard the word Prophy. So what is it? Prophy is short for prophylactic teeth cleaning. Most patients who require prophy are diagnosed by their dentist with an inflammation of their gums, commonly known as gingivitis. 

Simply put, it starts with poor brushing. The main cause of gingivitis is plaque and tarter build up. First comes the plaque. Plaque is the yellow film on your teeth caused by food and bacteria in your mouth. Next comes the tarter. When plaque buildup mixes with the saliva in your mouth it hardens over time and turns into tarter. Tarter, usually a result of poor brushing, ultimately leads to gingivitis. It’s a vicious cycle.

Gingivitis can also be a warning sign for a more severe gum disease called periodontal disease. Periodontal disease is when gingivitis is left untreated and can lead to bone loss or even tooth loss.

But don’t despair! The good news is that gingivitis can be treated with prophy. And with routine oral hygiene at home, such as proper and regular brushing and flossing, you can prevent gingivitis from reoccurring.

So how exactly does prophy work? Prophy is a multi-step process that is normally done in one office visit:

  1. First, a general cleaning is performed. This is either done manually or with an ultra sonic instrument called a cavitron, or sometimes a combination of the two. Both techniques loosen and remove plaque and tarter build up.
  2. Following the initial cleaning, a rotary machine that looks much like a toothbrush with toothpaste on it is used. This removes any remaining plaque around or under the gums. It also helps to polish the outer surface of your teeth which helps reduce future plaque build up.
  3. The final step is to floss to remove any and all remaining plaque between the teeth.
The risks associated with prophy are fairly minor. In some incenses during the procedure a Tooth Filling or Crown may become loose. Though keep in mind that any filling or crown that would become loose is most likely already defected. Any sound filling or crown will safely withstand the prophy procedure.

Tooth Filling Procedure

Cavities, as their name implies, are essentially holes in your teeth caused by decay. You can sometimes spot a cavity, as the area surrounding the hole generally looks dark brown or gray.


The main culprits to blame for your cavity are certain types of bacteria in your mouth. These bacteria are contained in plaque and they interact with the carbohydrates and sugars in your food creating an acidic environment that dissolves the protective enamel on the outer layer of your tooth. Once the acid succeeds in eroding the enamel, your tooth is exposed, leaving just the softer dentin layer, which will ultimately cause the formation of a cavity. At this point the decay process rapidly speeds up and spreads deeper into the tooth.

A Tooth Filling is necessary to treat your cavity because if left untreated the decay will eventually grow and will enter into your nerve canal. And yes, this can be as painful as it sounds. It can also lead to more serious problems such as infection or abscess.

A Dental Filling may also be needed to replace or repair an old worn down existing filling or to fix a chipped tooth.

In addition to potential pain and discomfort and possible tooth loss, if you wait to have your cavity filled it may end up requiring a more difficult process, such as a root canal, to save your tooth. And it could also cost you a lot more money to fix. The bottom line is you should get your cavity filled as soon as you can.
After your dentist detects a cavity, either through a clinical exam or with the use of X-Rays, it’s time to actually fill it. In most cases a local anesthetic is used to make the filling procedure virtually painless. Once the tooth is numb, the next step is to remove the tooth decay. This is done using a dental rotary machine as well as dental hand instruments. Once the decay has been removed, the tooth is now ready for the filling material of choice.

Composite is the filling material of choice when it comes to matching your tooth color. Composite Fillings consist of plastic and glass particles. They have become the most popular filling materials because they can match your existing tooth color and they also solidly bond or glue to the tooth. After the decay in your tooth has been cleaned, a cleansing gel is applied. Next, a bonding solution is applied finally followed by a composite filling material. Composite is hardened by applying high intensity blue light. It only takes a few seconds for the materials to harden. Once the tooth is filled and the composite is hardened, the filling is checked to ensure the proper shape and look. At this point any necessary adjustments are made and then your filling is complete.

Following the Tooth Filling Procedure, it is very common to experience some discomfort, normally at the site of the anesthetic or at the tooth itself. To alleviate the discomfort, it is recommended you take over the counter pain medication. If the symptoms persist, then you should visit your dentist.
In some cases the decay could be quite deep and close to the nerve of the tooth. In these instances the nerve could already be infected with bacteria. Even though a filling has been placed, there is still a good chance that the tooth may need to go under root canal therapy to relieve the discomfort.

Also, Composite Fillings may change color over time. Please keep in mind the longevity of a filling depends in large part on your oral hygiene and diet. So be sure to maintain your filling with proper brushing and flossing.

Primary Hemorrhage and its Treatment

Primary hemorrhage is postoperative bleeding that occurs immediately after an extraction. In essence, the wound does not stop bleeding. To permit clear visualization and localization of the site of bleeding, the mouth should be irrigated thoroughly with saline. The patient’s overall condition should be assessed. Once the general site of bleeding is identified, pressure should be applied for 20—30 minutes. Extraneous granulation tissue or tissue fragments should be carefully debrided. If the source of the bleeding is soft tissue (e.g., gingiva), sutures should be applied. If the source is bone, the bone may be burnished. Bee’s wax can be applied. Placement of a hemostatic agent, such as a surgical gel, in the socket may be followed by the placement of interrupted sutures. The patient then should be instructed to bite on gauze for 30 minutes. At the end of that time, coagulation should be confirmed before the patient is dismissed.

A clot may fail to form because of a quantitative or functional platelet deficiency. The former is most readily assessed by obtaining a platelet count. The normal platelet count is 200,000—500,000 cells/mm3 prolonged bleeding may occur if platelets fall below 100,000 cells/ mm3. Treatment of severe thrombocytopenia may require platelet transfusion. Qualitative platelet dysfunction most often results from aspirin ingestion and is most commonly measured by determining the bleeding time. Prolonged bleeding time requires consultation with a hematologist.

What are the generally accepted principles for cavity preparation?

1.     Cavity preparations should be governed by tooth anatomy, tooth position in the dental arch, extent of the carious lesion, and the physical properties of the filling material.

2.     Gingival margins should be ended on enamel whenever possible.

3.     Cavity preparation margins should be supragingival whenever possible.

4.     Margins of posterior cavity preparations should not end directly in occlusal contact areas. Contact areas should be composed of one material to allow for even wear. Uneven wear results if two materials meet at the contact area, thereby producing open margins.

5.     Weakened and unsupported tooth structure should be removed.

6.     Maintaining a dry work field with the use of a rubber dam is without equal and will always enhance the consistent quality of restorations.
